Making A Positive Difference

In residents Lives Through Servant Leadership

Our Story

Five Counties Nursing Home has an extensive history. The one consistent service that was established upon arrival of the railroad and has remained, is the availability of medical services.

November 15, 1951, the Five Counties Hospital opened, the newest, most modern hospital in a five-county area. The facility had 44 beds in total, two surgical wards, labor and delivery room, kitchen, laundry, housekeeping and 24-hour service.

On September 9, 1962, the nursing home addition was dedicated.

In 1988 after the Hospital closed years prior and some turn over in management, the community established a non-profit corporation, The Lemmon Area Medical Association. The corporation and its Board of Trustees saved the facility. To this day, the corporation still owns and operates the facility.

Our Mission

Five Counties is committed to preserving and improving the quality of health of the people we serve by meeting the needs of the whole person.

At five Counties, we value: Integrity, Excellence, Compassion, Respect, Stewardship, Teamwork, and Personal Growth. These values provide a standard of accountability that propel us forward in all our interactions.
